I have a question What is the SEND pathfinder?

Hey! So I'm doing an assignment on my level 5 senior practitioner course and im struggling with this question. I have researched, I have asked at work and nobody there seems to know.

I need to write about the SEND pathfinder information... when researching it my brain isn't taking in what it is, all i'm getting from it is that its a scheme that is supposed to improve support available for those with SEND. I know I sound really silly, and lazy but I manage to do everything else myself but this one just has had me completely stumped. Could somebody possibly please in one or two simple sentences tell me what the SEND pathfinder is so that I can make sense of what i'm reading to be able to answer the question myself.
