this can be done by an observation or by witnesses statment you can get childrens ideas and put them into your planning we do this with med term planning the children have a say on what they would like. also both can be answered by giving the children a choice i asked a child what they wanted to do child said painting i asked how they said doing finger paints so myself and the child set the actitity up i kept asking the child about colours, paperor card then i encouraged the childrens learning with language asking what they are doing? what colours thya are using, child came up with a painting of a play park and explained in detail what part of there picture was the swings , slides etc.

hope this helps
