I have cut and pasted Doodah's recent answer to a previous request:

When I wrote my reflective accounts, I wrote about what I did and matched it up to the performance criteria I was trying to achieve.

You will also need to reflect on the activity you carried out and write about what went well (eg you got lots of information about a particular child),

what did not go quite so well (eg the child began to notice you were observing them and started to behave differently),

how you could improve your practice (eg by using a different type of observation)

and what you would do differently next time (eg not make the observation quite so obvious).

Hope this helps.

and another one from xxclairexxx

All training providers have different forms for candidates to use as their reflective accounts.

the form should say its a reflective account form.

usually it will say

name, unit title and number and what element you are covering

activity/incident title

date of activity/ incident

what performance criteria covered

it will the say something like :
USE THIS FORM TO WRITE ABOUT ACTIVITIES OR INCIDENTS YOU HAVE BEEN INVOLVED IN.
SAY WHAT YOUR ROLE WAS, WHAT YOU DID AND WHY.
COMMENT ON THE OUTCOME.
COMMENT ON WHAT YOU COULD DO TO IMPROVE.

xx
