Things to look at would be:
-What is the transition? Is it gaining something or losing something? Is it horizontal (Eg: from home, to childminder to Nursery) or vertical (from pre-school into Reception)
-Attachments: is child losing anyone or gaining anyong? Eg: Starting pre-school may seem like losing mum/dad but they've got to deal with gaining a 'teacher' (key person).

-Changes to child way of thinking: so things like is it change in routine (usually nap in afternoon but now going to a Nursery), change in cultural expections (from home with Muslim expectations and roles to a multi-cultural childminder), change in identity(from who they are at home, to becoming 'student'), change in environment (moving house)? Look at socio-culturism and constructivism for this.

effects will be things such as:
-behavioural: if upset may become agressive or distructive, or may become distressed and become inward, shy etc.
-emotional-fear, anxiety, upset or may be happy, excited etc (not all transitions will have negative effect)
-physically-revert back to wetting bed, acting out like child.
-developmentally-support them in becoming more independent.
-intellectually-same as developmentally really-includes new learning opportunties
-identity-personality may change, name they refer to themselves, how they see themselves.
-spiritually-may be change in beliefs and understanding.

hope this kinda helps you x
