importance of early identification is so that support can be put in place at the first oppurtunity, i.e speach therapy, or specialised equipment, parenting classes etc to prevent further delays and remove barriers to development, or even a death
look at all agencies involved in development. gp, hospitals, teachers, social services, specialised services, all sharing information to identify areas of development and provide the best services. if you are in child care it could be how all agencies attend care team meetings or provide reports, a designated looked after nurse would carry out a health assessment and provide a report on areas of development could be diet, smoking, drugs and alcohol, you would then refer to this service, we all work together it depend on what age group you work with to how you answer this
Hope this helps a little
