Help SOS HELP!! please :) child involvement scale

Hi everyone, I'm currently in first year of the early years foundation degree and was wondering if anyone could help with my learning through play module.

I have to complete a adult and child involvement scale. I know the adult one has to be completed with 2 minute intervals with a description however the sheet I have for the child involvement scale observation guide it is a table with levels 1-5 with a description of what each level means and then a blank column, do I have to tick overall what level of involvement the children showed or do I have to write a description?
they are all put into an appendix and an essay is to be completed comparing and contrasting an adult led activity and a free play activity with little adult involvement.
