Help SOS Exit questionnaire?

I am interested to know whether anybody has an exit questionnaire they would share that they send out to parents when their child is leaving the nursery as ours is so dated and dull! Many thanks x

I'd be interested in this too. I have quickly typed up and re-printed one that was used in my setting a few years back, but it is in need of a bit of re-vamp. I am working to such a tight deadline to finish my level 3 I just had to go with what was already in place! I just thought something was better than nothing!

I wasn't familiar with this document and have also only just found out that they can extend to exiting staff - exit interviews

Does anyone offer or provide as a matter of course, stamped addressed envelopes for sending the questionnaires back?

i am really struggling trying to put together a exit questionnaire for children leaving nursery, i have done the exit questionnaire for staff. can anyone suggest any ideas please, anything would be greatly appreciated.

I have a basic one that we use but don't seem to be able to add attachments :(
