Default Survey format

Hi there wondering if anyone can help. Trying to think of an attractive way to set out a survey and the type of questions to go in it to see if parents would be interested in full day care instead of the morning sessions we currently offer. I don't want something they will get a few questions into and then get bored, can anyone help please Thanks:reading::idea:
