cyp 3.7 inclusion policy

My tutor has given me feedback on this unit asking for a inclusion policy and to add definitions for social model and medical medel of disability. DOES ANYONE KNOW WHAT SHE IS ASKING FOR PLEASE?
I thought I had covered this what am I missing? any ideas?
